All of these are real findings from checks we've actually run. The practices aren't named — yours wouldn't be either.

A “Patient Education” button on the homepage pointed at a domain that had since been re-registered by someone else. It was serving an overseas gambling site — to their patients, and to Google.

A Reviews page invited patients to “see what actual patients say” and then displayed nothing at all. The practice had 4.8 stars from 57 reviews. Not one of them appeared on their own website.

A homepage with a completely empty title tag, and a description still reading “Welcome to our Welcome page.” — the vendor's placeholder text, live for years, and the first thing Google showed anyone searching for them.

A site whose footer copyright named the marketing vendor rather than the practice, and linked to the vendor's own website. On a leased platform, that's not a typo — it's who keeps the site if you ever stop paying.
